The first 48 hours after lip filler treatment are the most critical for achieving the best possible outcome, and proper lip filler aftercare can make a genuine difference to how your results settle.

The First 24 Hours: What to Expect and What to Do
Immediately after lip filler treatment, some degree of swelling, tenderness, and possibly minor bruising is completely normal. Your lips may feel tight, look slightly uneven, or appear larger than your intended result. This is not the final outcome. It is your body's natural inflammatory response to the injection process.
What to do:
- Apply a cold compress gently. Wrap an ice pack or bag of frozen peas in a clean cloth and hold it against the lip area for 10 minutes at a time, with breaks in between. Do not apply ice directly to the skin, and avoid pressing firmly on the treated area.
- Stay hydrated. Drinking plenty of water supports the healing process and helps the hyaluronic acid filler integrate with your tissue.
- Eat soft foods. Stick to foods that do not require wide mouth opening or vigorous chewing: soups, smoothies, yoghurt, and soft pasta are all good options. Avoid anything excessively hot in temperature, as heat can increase swelling.
- Keep your head elevated. When resting or sleeping on the first night, try to keep your head slightly propped up with an extra pillow. This can help minimise swelling overnight.
- Take paracetamol if needed. If you experience discomfort, paracetamol is generally safe. Avoid ibuprofen and aspirin, as these are blood thinners and may increase bruising.
What to avoid in the first 24 hours:
- Alcohol. It dilates blood vessels and can worsen swelling and bruising.
- Strenuous exercise. Elevated heart rate and blood pressure can intensify swelling. A gentle walk is fine, but save the gym session for another day.
- Hot environments. Saunas, steam rooms, hot baths, and even very hot showers can increase blood flow to the face and exacerbate swelling.
- Touching or massaging your lips. Unless your practitioner has specifically instructed you to massage a particular area, keep your hands away from your lips. Pressing on fresh filler can cause it to migrate from the intended placement.
- Kissing and other pressure on the lips. This may feel like an unusual restriction, but applying pressure to newly injected filler can affect the final shape.
The Swelling Timeline: When Will My Lips Look Normal?
Understanding the typical swelling timeline is one of the most reassuring aspects of lip filler aftercare, because it helps you recognise that what you are seeing in the mirror is temporary.
Hours 1–6: Swelling increases steadily. This is the peak inflammatory window, and your lips may look noticeably larger than expected. This is normal.
Hours 6–24: Swelling typically reaches its maximum. Some clients experience more swelling in the upper lip, or notice one side appears slightly fuller than the other. Asymmetry at this stage is almost always temporary.
Days 2–3: Swelling begins to subside noticeably. Your lips will start to look more proportionate, though some puffiness may remain.
Days 5–7: The majority of swelling has resolved. Mild residual fullness may linger, but your lips should be looking much closer to their final result.
Days 10–14: This is when you can truly assess your results. The filler has settled into position, swelling has fully resolved, and any minor bruising has typically faded. This is why we recommend waiting at least two weeks before evaluating the outcome.
If your practitioner has recommended a follow-up appointment, it is usually scheduled around this two-week mark.
Days 2 to 7: Ongoing Aftercare
Once you are past the initial 24 hours, the restrictions ease, but some guidelines remain important for optimal lip filler aftercare.
- You can resume most normal activities, including gentle exercise, from day two onwards. High-intensity workouts are best left until day three or four.
- Avoid direct sun exposure and sunbeds. UV exposure can increase inflammation and may affect healing. If you are going outside, apply an SPF lip balm.
- Continue to avoid excessive heat, particularly saunas and steam rooms, for at least 48 hours.
- Keep the area clean. You can wear lip products from day two, but opt for clean brushes and fresh products to reduce the risk of infection at the injection sites.
- Stay hydrated. Hyaluronic acid-based fillers are hydrophilic, meaning they attract and bind water. Good hydration can help your filler look plump and natural.
- Avoid dental appointments for at least two weeks. The pressure and manipulation involved in dental work can affect newly placed filler.
Sleeping Position and Practical Tips
One question that catches many clients off guard is how to sleep after lip fillers. For the first two to three nights, try to sleep on your back with your head slightly elevated. Sleeping face-down presses your lips against the pillow, which can affect filler placement and increase swelling.
If you are not naturally a back sleeper, propping yourself up at a slight incline with pillows on either side can help prevent you from rolling over. It is only for a few nights, and it genuinely makes a difference.
A few other practical tips:
- Arnica cream or tablets may help reduce bruising. Many clients find applying arnica to the surrounding area (not directly on the injection points) helps bruises fade more quickly.
- Avoid straws for the first day. The pursing motion puts pressure on the lips.
- Be patient with asymmetry. Swelling rarely resolves at exactly the same rate on both sides. What looks uneven on day two is very likely to look balanced by day ten.
When to Be Concerned
While complications from lip filler are uncommon, particularly when treatment is performed by experienced practitioners, it is important to know the warning signs that require prompt attention.
Contact your clinic or seek medical advice if you experience:
- Severe, worsening pain that does not improve with paracetamol, particularly if it intensifies rather than eases over time
- Blanching or white patches on the lips or surrounding skin, which could indicate a vascular issue
- Disproportionate swelling on one side that continues to worsen after 48 hours
- Signs of infection such as increasing redness, warmth, pus, or fever
- Dusky or blue discolouration of the lip tissue
